DCMS Annual Awards

Each year, the Duval County Medical Society is pleased to recognize the volunteer, civic and leadership efforts of our member physicians.  Nominations are now being accepted for the 2009 Clyde M. Collins, MD Humanitarian Award, the 2009 DCMS Community Service Award, and the 2009 Philip H. Gilbert Young Physician Leadership Award.  Please note the general qualifications listed below and download the nomination form (available in Word and PDF format) for additional details.

The deadline for all award nominations is October 16, 2009.  Nominations will be reviewed by the DCMS Board of Directors at their November board meeting.  Winners will be recognized at the DCMS Annual Meeting on January 21, 2010.

2009 Clyde M. Collins, MD Humanitarian Award

The Clyde M. Collins, MD, Humanitarian Award was created by the DCMS in 1997 in memory of Dr. Clyde Collins to recognize volunteer efforts by physicians who exemplify Dr. Collins' caring disposition and commitment to the community he humbly served.  The DCMS Board of Directors reviews this award annually to determine if there is a worthy candidate.

2009 DCMS Community Service Award

Recognizes the outstanding volunteer efforts of DCMS physicians (either in active practice or retired).  The DCMS Community Service Award can be presented to any DCMS physician for volunteer work done outside of DCMS sponsored activities (e.g., excluding sports medicine pre-participation athletic physicals, etc.).

2009 Philip H. Gilbert Young Physician Leadership Award

Philip H. Gilbert served as the Executive Vice President of the DCMS from 1984 until his death in 2004.  During those two decades, he mentored many physicians in the art of leadership. Mr. Gilbert had a special talent for recognizing and developing leadership among young physicians.  The Philip H. Gilbert Young Physician Leadership Award was created to honor the memory and service of a good and talented man by recognizing Young Physicians with leadership traits that Mr. Gilbert would have admired.  Please see the nomination form for specific requirements.
